These Terms of Service are effective unless and until expressly terminated by Eleven Detroit IV Hydration and Medical Spa or you. You may terminate these Terms of Service at any time by notifying Eleven Detroit IV Hydration and Medical Spa that you expressly want to terminate your relationship with Eleven Detroit IV Hydration and Medical Spa and/or our Services or when you cease utilizing our website.

In the event Eleven Detroit IV Hydration and Medical Spa makes the determination, in its sole discretion, that you failed to comply with any term or provision herein, Eleven Detroit IV Hydration and Medical Spa make terminate this agreement at any time without notice and you will remain liable for all amounts due and owing up to and including the date of termination. Additionally, Eleven Detroit IV Hydration and Medical Spa may deny you access to our Services at our discretion.

THE OBLIGATIONS AND LIABILITIES OF THE PARTIES INCURRED PRIOR TO THE TERMINATION DATE SHALL SURVIVE THE TERMINATION OF THIS AGREEMENT FOR ALL PURPOSES.

SECTION 11 - Entire Agreement. These Terms of Service constitute the entire agreement and understanding between Eleven Detroit IV Hydration and Medical Spa and you with respect to your use of Eleven Detroit IV Hydration and Medical Spa’s Services. This Agreement has been entered into after full investigation. There have been no prior oral or written statements, representations, promises, understandings and agreements and all understandings and acknowledgements of the Parties are merged into and superseded by this Agreement, which alone fully and completely expresses their agreement.

If any provisions of these Terms of Service shall be unenforceable or invalid, such unenforceability or invalidity shall not affect the remaining provisions of this Agreement.

SECTION 12 - Governing Law/Venue.

This Agreement shall be governed by and construed in accordance with the laws of the State of Michigan. Should any action, suit or claim of any such aforementioned nature be brought by any party to this Agreement, such action, suit or claim may only be brought in the federal and state court located in the State of Michigan.

SECTION 13 - FDA

We use high-quality ingredients in our IV drips, including vitamins and minerals.  While the components are regulated by the FDA, the specific benefits of IV therapy have not been fully evaluated by the FDA.  These statements have not been evaluated by the Food and Drug Administration.  Our services are intended to support overall well-being and are not a substitute for a healthy lifestyle.  We recommend consulting with a healthcare professional before starting any new wellness regimen.

Some treatments may involve “off-label” use, which refers to the use of FDA-approved products in a manner that is not specifically listed in the approved labeling. This is a legal and common practice in medicine and is based on practitioner judgment and published clinical experience

SECTION 14 - PAYMENT, REFUND, AND APPOINTMENT POLICY

All procedures are via credit card, cash pay, or venmo. Full payment is due at the time of service. No refunds will be provided for treatment once administered. Touch-ups, if required, may incur additional charges.A 24-hour cancellation notice is required. No-shows or late cancellations may be subject to a 50% charge of services.
